Does anyone have UNICARE HMO and have this surgery approved?

I am from IL and have UNICARE HMO. I have searched UNICARE HMO under IL, and found that there are many people who list this insurance, and they are all "waiting for a reply"... I tried emailing some of them, and there either was no responce, or email has been changed... I was wondering if anyone here has UNICARE HMO and had their surgery approved..... Thanks! Laura
