Do most denials come from the insurance rather than the surgon?

I have been reading on here and noticed that most of the denial people encounter are with their respective insurance agencies. From what I hear mine is great about it (tricare prime) So I was wondering my biggest problem is lack of supervised diet history... what are the odds the SURGON will deny me? Thanks for your HELP!!!
