I have sent in my family doctors letters, my pych. evaluation, other doctors letters

My question is who gets in touch with the insurance co to see if they will pay? I see where other call their insurance co. My insurance is Medicare. When I called them they told me they don't pre-approve. I called my secondary insurance and they said they would pay whatever Medicare doesn't. So I don't know anything more then I did before. Will the doctors office handle this when they get all your papers?
