What kind of terms will make the Ins Co pay?

I appealed my TT and they denied it. I have lost 110 lbs and I am a diabetic. I also have a hernia which is covered but they will not cover my TT. I am so upset. I have Horizon BC/BS. I just don't understand. Can anyone suggest something?
