Can anyone help me with an appeal letter to United Health Care CHoice PLus

We submitted for a revision from my original RNY and was denied due to their exclusion.."surgical and non surgical treatment of obesity including morbid obesity" So now we are going to appeal..I need help with an appeal letter..thanks
