Do you know of any hospital that will do my surgery without pre-determination.

If any doctors are reading, how can i get a hospital to do my surgery without a predetermination. My insurance company does cover the procedure but wont provide a predetermination for anything at all. "not their policy" My hospital wants 25,000 dollars upfront and i cant pay. Please help me find a hospital that will help me!! Obesity Law told me they cant help. Insurance companys are allowed to do this. HELP!!
