Insurance wont approve unless my husband has medical records from 2 drs documenting

weight loss attempts. I had surgery under the same insurance plan almost 6 months ago and this was not needed for me. My insurance is Aetna HMO. My husband has diabetes, hypertension, high cholesterol, arithitis, and gout but has never been on any supervised diets. I did put together a diet history stating when drs have told him he needed to loose weight and that was submitted to insurance but they say they want medical records backing that up. The doctors dont have anything in writing in his medical records that state this that I can submit to insurance to get him approved. Can anyone please give me any advice as to how to get Aetna to approve him?
