Anyone ever abel to Get insurance to pay for a surgeon/hospial out of plan?

I live and work in Central Illinois. I am a nurse at OSF St. Francis Medical Center. I have OSF Healthplans HMO. I have done my research and do not really trust or want to have my surgery done at OSF. However, my insurance says that I can only have it done there. My room mate is having it in Moline, IL at Trinity Bariatric Center. I want to have it here also. The surgeon in Moline says I need to appeal to my insurance company because they can not limit me to only OSF. I am not sure what to do. I fear for my life by having it at OSF but am not willing to just not have the surgery. Any help would be appreciated. Thanks.
