Anyone had insurance approval for scar revision

I am wanting to get a scar revision. My RNY scar is keloided and has balls of fat necrosis under the skin that is painful. Anyone had insurance pay for this. My insurance paid for a TT and thigh lift and my RNY. I thik with the pain from the fat necrosis, I have a shot.
