If i do not have 12 supervised mth of diet history will i be denied by bcbs

I've already had my consult; the insurance corr. ask me to get 12mths of diet history from my pcp before she submits it to bcbs. I don't think my pcp has that. Will bcbs approve it without 12 mths? I think i may have about 6 mth. If i have my own list and have my pcp approve it will be enough for them? I nervous please help.
