Does any on have a letter from their orthopaedic dr

I have a orthopaedic surgeon that will write a letter for me but wants to see a sample letter. I have upper and lower back pain,and a hip injury that will eventually need a hip replacement. I am just looking for a sample letter from a dr so my dr can get a idea on how to write one. Can anyone help?
