7 weeks since Psych eval and he hasn't turned in report! I'm losing it!

I had my psych eval on March 5 (7 weeks tomorrow) and the psychiatrist has not turned the report into the surgeon. This is the last thing I need for my case to be submitted to the insurance. I've been calling the surgeon every day - they keep saying "he sent a bunch of reports, but yours was not in with them". I am getting so frustrated, I just don't know what to do! I've left 2 voice mails for surgeon, then spoke to him directly, emailed my surgeon (no response) and call his office daily! I am starting to feel very hopeless as I am in a time crunch to have this done before July at work. Any advice on what I should do would be greatly appreciated!!! Thanks.
