Need Help with the letter for Plastic Surgery

Hi there...I am in the process of meeting with a plastic surgeon. I would love, please, please, please, someones help in writing a letter that she could sign (or something close to it) that shows "reconstructive" needs as opposed to "cosmetic"...so that Aetna will possibly cover my PS. I have letters from my PCP, OB/GYn and Surgeon (Rny) but I know the PS will not write as good of a letter as I really need. HELP...please.
