I'm in need of a template of a letter used for coverage of a Tummy tuck!

Can anyone help me!! PLEASE I'm in need of a letter written by you to the insurance to try to get the Tummy tuck covered because of the hanging skin and other health issues. If anyone that has a copy of there letter and would let me use it as a template I would be very Greatful.
