Where can I find an insurance company that will cover 'just me'?

I'm self-employed, so there is no 'job insurance'. I'm looking for a company that will cover only me.... I've been trying to find a way to reach Aetna, or United Health Care, or Cigna because Dr. Fisher in Las Vegas said they cover WLS. Please help with ANY info. Thanks!
