looking for a surgeon that performs the lab banding.

Need a surgeon in the Buffalo,Rochester, Erie,Pa are that performs the lab band surgery. I have Comm Blue Ins and the doctor in Buffalo is not Credentialed with them and I have already had all my tests done back in Feb/March. Really would like to get this moving again.
