I cannot find on the site a good PCP letter for Aetna Us Healthcare

As I posted yesterday my PCP's letter just isn't god enough I was told by my surgeon, it was 1 short paragraph. I have tried searching this website to find one but can't seem to find a sample. Would anyone be able to e-mail me a sample of one that worked for you, if you have Aetna US Healthcare all the better. I have to call my PCP today aand let him know he needs to re-write it and I'm afraid, I know he's busy, and I think he's getting tired of hearing my name. Your help would be greatly appreciated.
