Does anyone have ANY information on General American Insurance???

Im so confused, weight for life is dealing with my carrier and told me today that i neither had a ppo or hmo and talked about in-networking and out-networking, im so confused??? What am i up against? if anyone has dealt with this company or this In or Out networking, please help me! Thanks
