has anyone ever had there medical records lost?

I am still researching surgery and have an appointment with the surgeon. But I have been reading a lot of post about having to summit medical documentation for 5 years. I am ex-military and they cannot locate my records. My current PCP only has my records for the last 18 months. Any suggestions? I have Cigna POS insurance Thanks in advance
