Can someone please help me write a good letter to my doctor?

I am needing to write a letter to the doctor I am going to use for ULS. The next consultation that they have isn't until June 2003. I have so many things coming up during the summer and I wanted to see if he will extend a professional courtesy to me. I work at the hospital he has his practise and on the same floor that he does surgery on. I understand I am not any better than anyone else. I wanted to start summer school to continue my nursing education and I would hate to be in the middle of two major things while working. If anyone has anything like this, please help me. Thanks in advance.
