What does the doctor write for medical leave

First of all I haven't told my work place about the WLS, I told them I may have to have my gallbladder taken out. So today I was given a form for my doctor to fill out. One question on the form is " explaination of extent to which employee is unable to perform the functions of his or her job" and another one " is the employee unable to perform work of ANY kind."I have to return this form to my office manager and she sends it to corp. office.... Now what do I do? I don't want to lie, but then again I don't want them to know the truth, it my personal choice. I don't want any one at my work to know the real reason, because they won't understand, or may not approve of me taking the time off, and I have to keep my job.... HELP....
