Can somebody send this chart to me?

Hi everybody! I am having my surgery in 6 days! Yahoo! I was sent a Excel weight loss chart by somebody from this website and I really want to use it but everytime I go to open it is says it cant open(A problem with my computer) Anyway could somebody please send this to me again? It is a chart that is in Excel and it calculates your BMI and weight as you go along. Thanks so much!!
