has anyone ever been given an excetions by their employer?

I have been denied twice through united healthcare ppo because of an exclusion my employer has. I work for a small company so i dont think they want to redo thier policy but i keep hearing about employers able to give a one time exception to their exclusion and I was just wondering if anyone has every had that happen and if so what kind of hoops did you have to jump through to get it. thanks all:)
