Pleze help me on the Co-morbities for Triwest (Tricare Prime) insurance

Please help I am starting over again. I was initally denied Gastric Bypass because I did not have any comorbities that Triwest (Tricare Prime outlined).(I was denied on the first appeal).Its been since April so I am starting over again, and I have my first appointment on 10 Jan. I want to make sure this time I am approved, any advice? Should I take pictures I need any advice. I can not find the comorbities that they outlined.
