help i am having a hard time

i am trying to make a consultation appt with surgeon in kansas city and have looked up info on this website and there is one other person who has the same insurance as me and got approved .. but the docs office said i need preapproval before consultation and my insurance carrier, principal, won't approve even with medical necessity! can anyone help me?? this is so depressing .. my bmi is 52% and i have all kinds of weight related problems and CAN'T do it alone
