is it possible to have a revision after gaining 80 pounds

I had RNY in 2003 i was 540 pounds the most i got down too was 245 at which i was happy with but in the past year i have gotten back up to 310 and i am very scared i need help or advise on what to do i went back to regimen but it seemed not to work
