anyone know of a surgeon that accepts payments on self pay portion of surgery due to

need oh,ky or wv,surgeon willing to accept payments on self pay portion of fees. my ins has a benifit cap of 12500. MD ,hosp,and anethesia fees combined desired around 10,000,-22000. May be able to go 23-24000 but that would be the limit. Any help or info appreciated. Thanks
