Condtions for insurance approval

I have just spoke to my doctor about having the lap band surgery. She told me that she would work on getting me a referl. I am a 100 pounds overweight and I have high cholesterol, I just told her that at the end of the day my feet hurt really bad. Great West is my insurance company, I am worried that I might not get accepted for insurance coverage beacuse it may not be medicaly needed. Has anyone else with Great west had the same lack of elments?
