How to lose weight again?

I had a baby in 10/04, and did great. I gained less than 25 pounds and my baby was healthy, considering I got pregnant 10 mos. out from WLS in 5/03. Here I am a year and a half later and having gained 40+ pounds and very seriously hating myself again. How can I jump start weight loss, get motivated to exercise (I don't want to leave my house because I feel horrible about the way I look and feel about my weight), and stop eating so much (I'm back on the "wheel"...you know, the cycle of hating yourself, eating to feel better, feeling worse for eating, and so on)? I'm extremely frustrated and depressed. Any thoughts or ideas?
