How long does it usually take to get a date for surgery

I have had my approval for 2 months and still no news for a date for surgery? I have spoken with the program manager every week and she has a different answer everytime I call... I feel like I've been pushed off, so should I take it a step further and go the the top where escaleted complaints go?
