Help! I got my approval in hand on Wednesday 15th.

I got my approval for surgery on July 15th called the doctor off and was told they have the approval in hand and they are a month behind on seeing patients due to computer problems. Okay my concern is on the approval letter it states and end date of Sept 16th. Is that the final day my approval will be valide? And, if so would I have to get approval all over again?
