Has anyone been denied by insurance for a repair of a gastro gastri fistula?

I have been denied and the Dr submitted a letter saying this is medically neccessary due to the complications of severe abdominal pain, vomiting, heart burn and other problems. And how or what is the best way to appeal this and possibly go thru.
