About Self Pay

i am in Portland Oregon and am having to pay for this myself. has anyone else been a self pay and i was wondering if they have you pay the whole amount up front or send you a bill. i was told here tat it would be around 33,000 and im going to be having it done at Good Samaritan hosptal. i am currently on month 4 of doing the food log/monthly weigh ins and have currently lost 30lbs and even though i dont have a date set yet i was told to look forward to it to being in january. i have been going though the obesity institute at the same hospital. Thank you for any info.
