I have UHC through my husbands insurance--it is choice plus--

i ahd the rny done in 2001 jan--i weighed 480lbs..I was down to 240 but now back up to almost 290..I want to have a revision done--does any one know if this insurance will pay and does anyone know the code for a revision procedure? I cannot say that I have done everything right, but I feel like a failure and I want a 2nd chance..
