Looking for info on Private Insurance

Could anyone give me some more information on the insurance that you purchased?....I have cigna pos and have been denied 3 times..due to not having 3 supervised professional attempts. even though I'm 5'3 and 320 lbs, BMI 56 with hypertension, sleep apnea, and a host of other things.....any information would be helpful...Veronica
