Does CIgna Cover TT , brachioplasty, thigh lift , breast lift ?

I had gastic bypass in 9/2004 , I lost 160 lbs and now have severe lower back pain , has anyone w/ cigna been approved for these surgeries, and does anyone recommend getting most of them done all at once ?
