is there any way around the 6mth nutritional documentation

I've been trying for 10 years to lose weight with diffrent doctors I didn't have a primary dr because of jobs without benefits. With cigna the procedure is covered but i don't have the documentation of my weight loss process plus with my thyroid there was no real progress
