Has anyone used Humana PPO

I just got Humana and don't really know how long I have to have it to have any medical procedures. I have my first appointment with the Surgeon. When I asked if they took Humana and what the requirments were they mentioned 3-6 months of doctor supervised weight loss attempts. I haven't been to the doctor in a few years, and a friend with the same insurance had the surgery and this part was waved due to her health and BMI. What's been your experience? Thank you!
