HS ANYONE HAD A POLICY EXCLUSION WITH BCBS OF LA/ PPO AND HAD THEM PAY FO

I HAVE BEEN TRYING FOR QUITE SOMETIME (YEARS) TO GET APPROVAL FROM BCBS OF LA. DUE TO A POLICY EXCLUSION THEY WILL NOT COVER AT ALL. I HAVE BEEN DENIED AND APPEALED & WAS DENIED. I NEED HELP AND ANY ADVISE WOULD BE APPRECIATED.
