getting approved

Can anyone please tell me how hard it is to get insurance companies to pay for WLS. I have High mark BC/BS. I haven't applied for it yet... Honeslty I'm scared to get turned down... BMI 47 with GERD, Sleep Apena, and Depression from all the diets gone bad! lol... Please help!
