Need Hints, tips, etc for getting insurance to pay for plastics

I am a single Mom and therefore money is not plentiful. My insurance Medical Mutual of Ohio Super Med PPO has been known to pay for tummy tucks, but I've not heard about the other stuff. Is there sample letters, or statement regarding how to get insurance to at least consider the medical necessity of such things? My friend got her breast lift, thighs, arms, and tummy done via her insurance but she doesn't know how she did it...meaning she didn't do anything special...her doc just submitted to her insurance. She didn't write any special letters just had pictures. So she's no help as she doesn't know a darn thing! LOL. Thanks!
