24yr/F w/BMI of 44 297lbs how do I get approval for PA Medicaid?

Does anyone know how I should start my journey towards WLS. Everytime I speak with my pcp about my weight concerns she tells me I need exercise. I know I'm 24 but I have health issues. I've had asthma all my life and it's very difficult for me to even brisk walk or do light exercise without having a very hard time breathing. Diabetes, high blood pressure, cancer and heart problems run in my immediate family. Where do I start towards getting approval for the surgery. Thanks for any help.
