Any discounted programs for people who can not get medical insurance

We are self employed & they will not approve me for medical insurance due to my BMI which is 48. I know I can finance but we do not have the best credit. We live in Sacramento California and I have even been looking on clinical trial websites. I need help and some guidance or someone to make my dream come true. I would love to have surgery but need help finding someone to do it maybe on a sliding scale. If you have any ideas or know a surgeon who has a big heart that would be wonderful.
