rehab facilities or nursing home with bariatric units

the 60 yr old patient can't go home because she cannot walk and therefore get to dyalisis, there are no mobile dyalisis services here in central florida. She needs a rehab facility or nursing home where she can get help to get mobile again while receiving dyalisis. Does anyone know of anything out there. I found one here in florida but they can't handle anyo
