How do you get insurance to pay for breast surgery?

I just read Carnie's book. Carnie Wilson said if your skin is sagging and rubbing raw areas you might qualify for plastic surgery paid for by your insurance. So my question is" my boobs are going to be totally saggy when I loseweight as I ballooned up from34c to 42DDDwhen I gained so much weight. So do I have to go braless in order TO show it will rub raw? If I wear a bra the skin wont rub.
