Up-front fee for a revision...

I am going for a revision, had Lap Band removed in Dec 2007. I saw the surgeon yesterday, his office said that no matter what insurance you have their is an up-front fee that might or might not be covered by your insurance. I live in California.... has anyone heard of this?
