Has anyone with Pacificare prepared a diet history they accepted?

My PCP referal nurse says I have to pick up a diet sheet from my Dr. Office and be weighed each month for a year before they will refer me to a surgeon. I have documetation of being on diet pills twice and attending the rader institute. But she says that does not count. Does she know what she is talking about?
