Sick, sick, sick...First appeal denied, need ammo for second appeal.

I am absolutely sick to my stomach. My insurance has denied my first appeal. And it was a bang-up job I have to admit myself. I must start immediately working on the second appeal and don't know really where to start. In my first appeal, all aspects of my (poor)health were covered and I addressed the out of network benefits question, they don't have a surgeon in network that does the procedure my physician and myself are sure will work for me. When I orginally put in my request WLS was covered, now they are saying after July 1, 2001 they will not cover any obesity surgery......need some encouragement and inspiration here...
