I have BC/BS PPO. What will I have to pay out of pocket

What will I have to pay out of pocket at the time of my surgery? I haven't met my deductible yet. I am about $55 short of that. I know that I will be responsible for up to $1,000 if my 10% of the bill is that much. Is there any sort of down payment required? For the hospital or the doctor?? I have to plan ahead just in case?? I would think they couldn't bill me until after the surgery because there may be additional costs. Any input would be helpful and thank you in advance.
