Insurance Denial with Aetna ppo. How to appeal?

I have a girlfriend who was just denied by Aetna PPO because they said that they don't think she tried enough avenues in losing weight. She is going through Bariatric Treatment Center (BTC) and they automatically appeal for you. The question is....Is there anything she can do in the meantime. Like sending Aetna a APPEAL letter HERSELF? If so, does anyone have an example appeal letter they can send me.
